Breast Cancer and the Environment:A Life Course Approach

WASHINGTON — Women may be able to reduce their risk for breast cancer by avoiding unnecessary medical radiation, forgoing use of combination estrogen-progestin menopausal hormone therapy if possible, limiting alcohol consumption, maintaining a healthy weight, exercising regularly, and avoiding tobacco use, says a new report from the Institute of Medicine. These preventive steps focus on the environmental risk factors for which there is consistent scientific evidence of an association with breast cancer. 

The evidence also indicates a possible, though currently less clear, link to increased risk for breast cancer from exposure to benzene, 1,3-butadiene, and ethylene oxide, which are chemicals found in some workplace settings and in gasoline fumes, vehicle exhaust, and tobacco smoke.  Avoiding personal use of hair dyes and non-ionizing radiation emitted by mobile devices and other technologies likely will not impact a woman's risk for breast cancer, as multiple studies have found no connection to the disease.

Because of insufficient or contradictory evidence, the scientific jury is still out on whether many chemicals of concern, including bisphenol A (BPA), pesticides, ingredients in cosmetics and dietary supplements, and other substances alter the risk for breast cancer, the report says.  Women may choose to minimize their exposure to some chemicals, but the committee found the research inadequate to draw conclusions about the potential benefit of such actions.  Chemical ingredients in cosmetics, dietary supplements, and other products undergo only very limited testing before they are put on the market, and the committee noted the value of efforts to help consumers become more aware of this issue.
The steps identified in the report have the potential to reduce risk for breast cancer among women in general, but the committee cautioned that the evidence on how much risk reduction any of these steps offers is inconclusive.  Whether it is small or significant, the impact on individuals will vary considerably because women are exposed to a range of substances throughout their lives; in addition, biological, physical, and genetic factors influence their individual chances for developing the disease. 
The report's conclusions are the result of a detailed review of scientific research on environmental factors that may affect breast cancer risk.  The committee also explored challenges to studying possible links between environmental exposures and breast cancer and recommends future research directions.  Areas where there is provocative but as yet inconclusive evidence and that warrant priority attention include overnight shift work and accompanying disruptions of the sleep cycle; chemicals that mutate genes, alter gene expression, or affect hormones such as estrogen; and gene-environment interactions.  More research needs to be conducted on the effects of exposures throughout the entire life span, including at specific stages of breast development, and on the cumulative effects of exposures at different life stages or multiple exposures that occur together, the report emphasizes.  Most research has focused on adults and on exposures occurring within a few years prior to a diagnosis, but recent studies have shown the importance of exposures at various life stages, such as childhood, adolescence, pregnancy, and menopause.

In many cases, more information also needs to be gathered to determine whether preventive steps can be taken and how they can be most effective.  For example, we do not yet know when weight loss is most likely to be beneficial in reducing postmenopausal cancer risk.   

"Breast cancer develops over many years, so we need better ways to study exposures throughout women's lives, including when they are very young," said committee chair Irva Hertz-Picciotto, professor, department of public health sciences, and chief, division of environmental and occupational health, School of Medicine, University of California, Davis.  "We also need improved methods to test for agents that may be contributing to breast cancer risk and to explore the effects of combined exposures."

India.Advanced Project Preparedness for Poverty Reduction-Supporting Clean Village Environments

On 31 August 2009, ADB approved the provision of a Technical Assistance Cluster (C-TA) to India for Advanced Project Preparedness for Poverty Reduction to the Government of India. The C-TA is financed on a grant basis through the Department of International Development (DFID), funded by the Government of the United Kingdom, under the DFID and ADB Partnership for India, 2009-2013. The C-TA is implemented until 31 December 2013 in two phases: The first phase (Phase 1), in the amount of $14 million equivalent, covers 17 component TA projects with completion by 31 August 2012. The second phase (Phase 2), approved through a major change in scope and implementation arrangements by the ADB Board on 4 October 2011, entails an increase of $8 million equivalent and the addition of 9 new component TA projects to be completed by 31 December 2013.

Tanzania.Water Sector Development Programme Project

This Environmental and ad social management framework aims to ensure that the project activities of the Tanzania Water Sector Development Programme Project are carried out in an environmentally and socially sustainable manner. This report is organized in seven chapters.

Chapter 1 provides background information. Chapter 2 provides the relevant Tanzanian environmental policies and legislation applicable to the project and the relevant operating safeguards and policies as well as gaps that exist. Chapter 3 describes likely social and environmental impacts during each of the project phases. Chapter four gives a step-by step presentation of the screening process for sites of future activities. Chapter 5 gives the environmental monitoring plan. Chapter 6 gives the capacity building and training requirements for implementing the ESMF. Chapter 7 gives the recommendations of the study. The project's civil works are expected to affect forests, animals, and other natural resources, as well as affect communities physically ( air and water pollution, nuisance, and contamination, etc.); and socio-economically (land use, income generation, mobility, and community association). Water supply services will require more water abstraction, resulting in ground and surface water regimes; and will increase waste water generation, water stagnation and sanitation problems. Rehabilitation works will require demolition of the existing infrastructure and will generate rubble and waste needing disposal.

The increased numbers of people on the project site will deplete natural resources, pollute public waters and lead to soil degradation. Increased interactions among people will result in social and health problems stemming from increased disease transmission, and added pressures on social and health (medical) services.

Environment: Major expansion of Europe's protected natural areas

European Commission-Press release. Brussels, 21 November 2011. Natura 2000, the EU's network of protected areas, has undergone a significant expansion. Nearly 18 800 square kilometres have been added, including a major addition of marine areas covering 17 000 square kilometres which will increase protection for many endangered marine species. The network now covers almost 18% of the EU's landmass and more than 145 000 km² of its seas. The main countries involved in this latest expansion are the UK, France, Belgium, Greece, Cyprus, Hungary, Lithuania and Italy. Natura 2000 is the centrepiece of Europe's battle to halt biodiversity loss and safeguard ecosystem services.

Janez Potočnik, European Commissioner for the Environment said: "Natura 2000 is at the moment one of the most effective tools we have in Europe to combat biodiversity loss, and it plays a key role in our strategy to protect our natural heritage. I particularly welcome the improved coverage of European seas: protecting Europe's marine environment and its unique features has never been more important."

Natura 2000 is a vast network of nature conservation areas set up to ensure the survival of Europe's most valuable and endangered species and habitats. The network consists of around 26 000 sites, and the latest additions add 166 new sites covering nearly 18 800 square kilometres. More than 90 % of the area added is made up of marine sites (17 000 km²), mainly in the UK, but also in France, Belgium, Greece, Cyprus and Italy.

The new marine sites will provide a vital refuge for many of Europe's rarest and most endangered species. In the Atlantic, the UK additions feature 9 coldwater reefs, including reefs off Rockall Island which are biodiversity hotspots home to coral, sea spiders and numerous as yet unnamed species. In the Mediterranean, the new sites will improve protection for emblematic species such as the green turtle Chelonia mydas, the loggerhead sea turtle Caretta caretta and the Mediterranean monk seal Monachus monachus, which play a key role in the ecosystems they inhabit.

The expansions will also increase protection for a range of valuable terrestrial habitats, including peat bogs in Lithuania, salt plains in Hungary, and species-rich chalk grasslands in Italy and Cyprus.

The adoption of these Commission Decisions marks an important step towards finalising the establishment of the Natura 2000 network by 2012, a key activity included in the range of proposals in the new EU Biodiversity Strategy adopted by the Commission this year.

Background

Natura 2000 is a network of protected areas, consisting of Special Areas of Conservation established under the EU Habitats Directive and Special Protection Areas established under the EU Birds Directive. Natura 2000 is not a straight jacket: activities such as farming, tourism, forestry and leisure pursuits can still be carried out inside the network as long as they are sustainable and in harmony with the natural environment.

Member States choose their Natura 2000 sites established under the Habitats Directive in partnership with the Commission, and once selected, the areas are formally recognised by the Commission as "Sites of Community Importance" as has happened today. This process confirms the formal status of the sites, and cements the obligations to protect them. Member States then have six years to put the necessary management measures in place and designate the sites as Special Areas of Conservation.

The latest update concerns fifteen Member States, and increases the number of "Sites of Community Importance" by 166. The recent additions cover six bio-geographical regions – the Alpine, Atlantic, Boreal, Continental, Mediterranean and Pannonian regions.

The range of protected areas is vast, from flower-rich meadows to cave systems and lagoons. The nine bio-geographical regions of the network reflect the wide variety of the EU's biodiversity.

Biodiversity – the limited resource that is the variety of life on earth – is in crisis. Species are being lost at an unprecedented rate as a result of human activities, with irreversible consequences for our future. The European Union is combating this and recently set itself a new objective of halting biodiversity loss in Europe by 2020, protecting ecosystem services such as pollination (and restoring these services where they are degraded), and stepping up the EU contribution to averting global biodiversity loss. Natura 2000 is a key tool needed to reach that objective.

Approaches for Ecosystem Services Valuation for the Gulf of Mexico After the Deepwater Horizon Oil Spill: Interim Report (2011)

The unprecedented magnitude of the Deepwater Horizon oil spill presents significant challenges for oil spill responders and those tasked with assessing the impacts of the spill. Evaluating changes to ecosystem services—the benefits people receive from natural resources and processes—caused by the oil spill could expand the potential to capture and value the full breadth of impacts to the ecosystem and the public. This report assesses the methods and metrics that could help scientists effectively evaluate ecosystem services.
Key Findings
  • The Oil Pollution Act of 1990 establishes a formal legal framework for determining when an oil spill results in an "injury"-- defined as an observable or measurable adverse change or impairment in a natural resource. This requires assessment of the extent and severity of an injury to a natural resource through a process known as the Natural Resources Damage Assessment (NRDA). In addition to quantifying the extent of damage the assessment includes plans for developing, implementing, and monitoring restoration and compiles expenses for both assessment and restoration costs from those deemed responsible.
  • The magnitude and depth of the Deepwater Horizon oil spill, together with the inherent complexity of the Gulf of Mexico ecosystem, pose serious challenges to those charged with assessing damages and developing restoration plans. For a spill the size of the Deepwater Horizon, an "ecosystem services approach" may complement the Natural Resources Damage Assessment and offer a broader opportunity to capture, value, and appropriately restore the full breadth of impacts to the ecosystem and the public. This approach would focuses not only on replacing or repairing damaged natural resources but also on re-establishing or replacing the ecosystem's interdependent processes.
  • In order to assess the impacts of the Deepwater Horizon oil spill, scientists need to establish a baseline of ecosystem conditions before the spill took place. However, there are natural variations in conditions in the Gulf of Mexico over time, as well as human-made changes to the environment. Analysis of the impact of the Deepwater Horizon oil spill will need to take into account the fundamental complexity of the Gulf of Mexico ecosystem, and the past and ongoing affects of phenomena, both natural and human-induced, that are unrelated to the Deepwater Horizon oil spill.
  • Implementing an ecosystem approach to damage assessment requires an understanding of the complex linkages amongst various ecosystem components, including the impact of humans on the structure and function of the ecosystem, the resulting changes in ecosystem services, and how these changes affect human well-being.
  • Determining the impact of human actions on the structure and function of the ecosystem
    Scientists will need to investigate each ecosystem service by carrying out specific types of sampling and analysis to complement the information collected under the existing damage assessment process. In order to extend the current damage assessment to include an ecosystem services approach, scientists need to understand how these various components have been affected by human actions, the consequences for the structure and function of the ecosystem, and ultimately the changes in ecosystem services caused by the spill.
  • Establishing how changes in the ecosystem lead to changes in ecosystem services
    This step determines ecosystem production functions –how the ecosystem transforms inputs into outputs such as the yields of crops or fisheries. For many ecosystem services, a lack of mechanistic understanding and data inhibits accurate quantification of ecosystem services. The complexity of marine ecosystems makes it difficult to understand how disturbances to an ecosystem will reverberate through the system and ultimately lead to changes in the provision of ecosystem services.
  • Establishing how changes in the provision of ecosystem services affect human well-being.
    The third component of the ecosystem services approach focuses on establishing the value of ecosystem services. This involves combining economic methods with ecological assessments to estimate the value of changes in ecosystem services as a result of environmental impacts. Valuation methods are used to provide a common, quantitative measure to facilitate comparisons among various services as an indication of how much the availability of the service contributes to the improvement in human well-being. Alternatively, in the case of damage to the environment, valuation methods could be applied to assess how much value has been lost as a consequence of reduced ecosystem services.

The Slovak Republic should integrate environment into its economic development

As the Slovak Republic strives to increase productivity and competitiveness in the recovery from the financial crisis, the OECD Environmental Performance Review of the Slovak Republic recommends that it strengthen environmental policies and institutions, promoting green growth to help achieve its economic goals.
The Slovak Republic has made major progress in protecting the environment and enhancing its citizens’ quality of life over the past decade. Its environmental achievements include: reducing emissions of most air pollutants during a period of fast economic growth; significantly improving the efficiency of water, energy and material use; improving industries’ environmental performance; and promoting biodiversity conservation in central Europe.

The report also identifies a range of challenges: unhealthily high pollution in some urban centers; dangerous and costly floods; high reliance on landfill for waste disposal; and among the highest consumption of energy and carbon per unit of GDP of any OECD country.
The OECD Environmental Performance Review of the Slovak Republic makes 35 recommendations including:
  • Develop a new environmental strategy as an integral part of Slovakia’s economic and social development planning.
  • Improve general innovation capacity, including promoting eco-innovation.
  • Draw up a coherent framework to develop and implement climate, energy and transport policies.
  • Make environmentally-related taxes more efficient and effective.
  • Strengthen dialogue and co-operation with business, NGOs and other stakeholders.
  • Better target environmental outcomes when designing support schemes for agriculture and rural development.

Mexico Forests and Climate Change Project

Wordl Bank. Project Development Objective is: to support rural communities in Mexico to sustainably manage their forests, build social organization, and generate additional income from forest products and services including the Reduction of Emissions from Deforestation and Degradation (REDD+).

The project would help consolidate and improve CONAFOR#s incentive programs for community forestry and environmental services, and utilize them as key elements of the national REDD+ strategy. It would also help strengthen CONAFOR as a world-class forest agency, promote the alignment of rural development policies and programs, and pilot innovative REDD+ approaches in two Early Action areas.

Component 1. Policy Design and Institutional Strengthening. (IBRD US$30m, and FIP US$11.66m grant). This component would aim to strengthen CONAFOR as a leading forest agency worldwide, foster cross-sector collaboration among public agencies, and improve the quality of private technical assistance available to communities.

Component 2. Consolidation of Priority Community-Based Programs at National Level.(IBRD US$270m). This component will continue and scale up previous successful Bank engagement in community forestry and payments for environmental services with Mexico. It will support demand-driven community-based sub-projects related to social organization, capacity-building and land-use planning, as well as the protection, sustainable management, harvesting, processing and marketing of forest goods and services. Support to selected communities would be provided in the form of grants following the existing CONAFOR procedures (reglas operativas) which are reviewed and updated annually.

World Bank. China: Second Beijing Environment Project

Ratings for the Second Beijing Environment Project for China were as follows: outcomes were satisfactory, risk to development outcome was low, Bank performance was satisfactory, and borrower performance was also satisfactory. Some lessons learned included: the Bank should adopt a flexible approach to the institutional model for the sector, and in particular consider the country conditions, instead of advocating the traditional utility model as the default option.

The arrangements in place in Beijing assure sustainable service provision, which rely on a combination of politically feasible tariffs and regular confirmed resource transfers from the municipal government. However, such an arrangement is only feasible where the city can: (i) ensure service provision at low cost; (ii) take on responsibility for servicing debt; and (iii) provide timely cash transfers to meet shortfalls, especially in relation to new investments. Water re-use in water scarce areas is sound policy. Beijing has performed well to achieve a 50 percent rate of water reuse, by addressing the key issues relating to water re-use: (i) reclaimed water quality and reliability; (ii) price of reclaimed water; and (iii) cost and benefit of using water supplied through the system and reclaimed water. This should serve as an example for other Chinese cities.

The boiler conversion program faced not only major technical, environmental and economic issues, but also significant coordination and political issues. The ambitious targets set for conversion to natural gas-fired boilers were only achieved (albeit mostly outside the project) because of strong leadership, both at central and municipal government levels. In areas involving significant commercial operations (such as boiler conversions), newly established municipal entities with no commercial expertise are not likely to succeed. It might have been more appropriate for the government to have partnered with a truly commercial company, or the boiler conversions might have been left entirely to market forces with some incentives to end-users.

Environment: OECD supports Israeli initiative to develop a green growth plan

This was the main recommendation of the OECD Environmental Performance Review of Israel. The report is the first OECD review of Israel’s environmental policies since the country joined the OECD one year ago. Presenting the report to Prime Minister Benjamin Netanyahu, OECD Deputy Secretary-General Rintaro Tamaki commended the Israeli government’s recent decision to prepare a green growth plan.

As a relatively small, water-scarce, densely populated, and highly-urbanised country, Israel faces pressing environmental challenges. It has made good progress in addressing air pollution, water use and some other environmental challenges.

In the process, Israel has developed a dynamic clean-tech sector. However, the fast pace of economic and population growth intensifies pressures on the environment, including waste generation, habitat degradation and greenhouse gas emissions.

This Review presents 41 recommendations to help address these challenges including:

  • Continue to expand the use of environmentally related taxes and market-based instruments, and gradually remove tax concessions that are potentially harmful to the environment.
  • Strengthen the mix of policies to support the marketing and diffusion of environment- and climate-related technologies.
  • Reinforce environmental liability for damage to natural resources.
  • Gradually increase the agricultural and industrial sectors’ share in financing the full costs of water infrastructure.
  • Set up a system to monitor greenhouse gas emissions and provide an annual assessment to the Parliament on progress in achieving targets.
  • Review current arrangements to manage waste and draw up a comprehensive new policy, possibly including new legislation and an action plan.
